As a participant or legal representative of a participant in any or all activities organized by EDUNET SPACE SRL, CUI 52158020, at Strada 1 Decembrie 1918 nr. 90, Târgu Jiu, hereinafter referred to as “EDUNET SPACE”, by this agreement I declare and assume the following:
1. Identification details
I declare that all my identification data, as well as those of the minor for whom I am a parent or legal guardian, are complete and accurate. EDUNET SPACE reserves the right to request an identity document to verify the data of minors over 14 years old and/or of the accompanying parent or guardian.
2. Acceptance of this agreement
I confirm that I have read and fully understood each provision of this agreement and I expressly accept it in its entirety, agreeing with its content and effects.
3. Acceptance of EDUNET SPACE regulations
I confirm that I have read and fully understood the participation rules for activities organized by EDUNET SPACE, available on the website in the
Terms and Conditions section, and I agree with their provisions and effects. I have explained these rules to the minor according to their age and level of understanding.
4. Personal data processing notice
I confirm that I have read and understood the data processing information notice, available on the website in the
GDPR Policy section, and that I agree with its content and effects.
5. Compliance with instructions
I undertake to carry out, and to ensure the minor carries out, activities organized by EDUNET SPACE in compliance with applicable regulations, the instructions of EDUNET SPACE staff/collaborators, and the instructions displayed on-site for each activity.
6. Removal from location
I understand that if regulations or staff/on-site instructions are violated, I may be removed from EDUNET SPACE, and the minor may also be removed, without ticket refund.
7. Physical and mental fitness for participation
(1) I declare that I meet the minimum age required for participation, namely at least 2 years old for minors, and that my physical and mental health, as well as physical condition, allow participation in activities organized by EDUNET SPACE. For the minor, I declare that they have the health and physical condition required for participation, including discipline level and risk self-assessment capacity appropriate to age.
(2) I declare that I am not under the influence of alcohol, drugs, narcotic substances, or other substances/medication that may affect physical or mental capacity.
(3) I declare that I do not suffer from conditions that would prevent participation in activities carried out within EDUNET SPACE or that could generate disproportionate risk for me or third parties.
(4) I understand that certain conditions may cause discomfort or risks during activities, including but not limited to: fainting, dizziness, loss of consciousness, hypertension, chronic bronchitis, severe asthma, rheumatic fever, pneumothorax, liver or lung diseases, thyroid or adrenal gland disorders, other endocrine disorders, chronic bone conditions, arthritis, blood diseases, severe anemia, chronic ear or sinus diseases, previous severe cranial trauma, or any condition requiring regular medication.
I declare that I understand I am responsible for evaluating my own health condition or that of the minor and that, if such conditions exist, medical approval is required before participating in activities organized by EDUNET SPACE.
